Diabetes is a very common condition that many people can find difficult to properly manage. It can also be very hard for people to know exactly what services can be used for diabetes care in their local community. The Diabetes Out West website was developed to overcome these issues for residents in the Maribyrnong, Brimbank, Melton and Wyndham councils. This website has important information on diabetes, the type and cost of support services and where to find them. The use of this website empowers people in these councils to be more proactive in their diabetes care rather than solely relying on their doctors to make decisions.
