Joining up the care
HealthWest Partnership brings together 54 health care providers, community organisations and local government members in Melbourne’s west. HealthWest also seeks voluntary strategic alliances with organisations such as Victoria University and other relevant community services. These alliances strengthen coordination and input into public health policy, prevention, early intervention and chronic care initiatives.
