HealthWest Partnership and Inner North West PCP (INWPCP) have partnered with the National Disability Service (NDS) to bring you the Western Melbourne Cross-Sector Community of Practice.
The Community of Practice brings together people from the health, community, aged care and disability sectors to collaboratively identify and respond to the emerging challenges and opportunities arising from the NDIS, Aged Care and Mental Health reforms. We aim to exchange good practice to achieve better consumer-directed-care outcomes.

Who Can Attend?
Participation is open to any clinician/worker, team leader or manager representing services that are interested in progressing a consumer-directed care approach within their organisations across the west.
Why Come Along?
These forums will assist participants by providing examples of consumer-directed care and innovation, while offering a great opportunity for networking and collaboration. See the Terms of Reference.
